A Living Donor Hepatectomy is a surgical procedure where a healthy individual donates a portion of their liver to a patient suffering from end-stage liver failure. Who Can Be a Living Donor?

To ensure the safety of the donor and the success of the transplant, potential donors must meet rigorous physiological and psychological criteria. General requirements include:

  • Must be a willing volunteer between the ages of 18 and 55.
  • Must possess a compatible blood type with the recipient (though ABO incompatible protocols exist, matching is preferred).
  • Must be in excellent physical and mental health with a normal Body Mass Index (BMI).
  • Must have a liver of adequate volume and excellent anatomical structure without fatty liver disease.
  • Must be free from chronic illnesses such as diabetes, heart disease, cancer, or viral hepatitis.

The Intensive Evaluation Process

Potential donors undergo a meticulous, multi-phase evaluation over several days. This is designed to identify even the slightest risk factor. The evaluation includes:

  1. Phase 1 - Initial Screening: Blood typing, basic metabolic panels, and psychological screening to ensure the donation is entirely voluntary and not coerced.
  2. Phase 2 - Advanced Imaging: High-resolution CT scans and MRIs with 3D volumetric reconstruction. Dr. Reddy uses these scans to precisely calculate the volume of the liver, ensuring that the piece left behind for the donor is more than sufficient. The scans also map out the intricate biliary and vascular anatomy.
  3. Phase 3 - Comprehensive Medical Review: Cardiac stress tests, pulmonary evaluations, and final clearance by an independent Donor Advocate team whose sole responsibility is protecting the donor's best interests.

Advanced Surgical Techniques

Depending on the recipient's needs, Dr. Reddy will perform either a Right Lobe Hepatectomy (usually for adult recipients, removing ~60% of the donor's liver) or a Left Lateral Sectionectomy (usually for pediatric recipients, removing ~20% of the donor's liver).

Pioneering the Waterjet Technique

Dr. Reddy is highly proficient in utilizing the Waterjet resection technique for donor surgeries. This technology uses a highly pressurized, hair-thin jet of water to delicately wash away liver cells (parenchyma) while leaving the vital blood vessels and bile ducts completely intact. This leads to incredibly precise anatomical division, drastically minimized blood loss, and significantly faster recovery for the donor.

The Donor's Recovery Journey

Following surgery, donors typically spend one night in the ICU for close monitoring before moving to a specialized transplant ward. The total hospital stay is usually 5 to 7 days.

Most donors can resume normal, light activities within 4 weeks and return to work within 6 to 8 weeks. Because the liver regenerates so quickly, donors do not need to take any long-term medications or adhere to strict lifelong diets. They can continue to live a completely normal, healthy lifespan, carrying the profound knowledge that they have saved a life.
